• Packet intake stores support needs, county, payer information and timing for eligible paid packets. Records are limited to the signed-in household and authorized administrators. • Starting guide stores selected answers and the assigned route when configured questions and routes exist. An email added after the result is stored only with that response, with no subscription or email send. Without questions and routes the guide collects no responses. Retention period not yet set. • Care week cost stores anonymous chosen inputs, rates, sources, years, results and handoff clicks when sourced benchmark data and valid configuration exist. • Payment identifiers support the Verified Care Packet and organization claim. Checkout is not currently offered while payment configuration is unavailable. If enabled, Stripe hosts payment entry; Daymark stores payment event, checkout and authorized organization customer identifiers. Retention period not yet set. Other records and limits

Public corrections are stored for review. Share public information only. Do not submit patient names, diagnosis or medical details. Retention period not yet set.

Document uploads are on hold and are not currently offered. There is no self-service delete or export yet.

Packet-received and packet-ready transactional mail is supported only when configured. Delivery status is logged separately. Messages contain no intake details or private notes. Unconfigured mail stays unsent. Retention periods for delivery and sanitized integration-error logs are not yet set.

Access, correction and deletion

A direct channel for private access, correction and deletion requests is not currently offered. The public directory update form is not a privacy-request channel.

Cookies and usage information

Sign-in uses an essential session cookie. The app sends page path, referrer, screen size, browser language and available page-load timing to its own usage endpoint. It also reports application errors for troubleshooting. These are not descriptions of medical care or intake notes. A retention period for usage and error reports is not set in this app’s configuration.
