Easter Seals Case Management services keep individuals with disabilities and their caregivers on track to get the services they need, such as therapy, durable medical equipment, and resources for food, housing, and legal assistance. Services are offered under three categories: Community Living Assistance & Support Services (CLASS) Case Management, Veterans and Families, and Navigation.

For more information, email the Case Management team at casemanagement@eastersealshouston.org. For clients ineligible for the above three case management programs, Easter Seals provides limited information and referral services.
